Customer Forms

SP Images makes it simple and easy for new customers to open an account and existing customers to update their information. Due to PCI compliance regulations as wells as our concerns for individual and business privacy only some of our forms are available to be completed online. As a Wholesale Distributor, SP Images requires a copy of your Resale Tax ID for tax reporting purposes. Please have a PDF, JPG, PNG or other image file of this document saved to your computer or storage device as you will be required to upload a copy before submitting your application. SP Images also requires that all customers paying for purchases with a credit card complete and sign our Credit Card Authorization Form. Due to PCI compliance regulations credit card information cannot be collected via an online form. Thus, the form must be downloaded below and completed. You may upload the completed and signed form to SP Images directly using our Document Upload Sevice or you may submit the form in several other ways as outlined at the bottom of the form. Customers with a business located in Massachusetts or Ohio must also complete, sign and submit the below designated tax form. If you prefer to download or print a hard-copy of any form you may send the completed form directly to our Corporate Office at the address or fax number below.
